Whenever a person meets with an accident, the first worry of everybody is to make sure that there is no fracture in the bone. One way to find this out is to look for inflammation. If there is significant swelling and pain then you should immediately head to the emergency department of a hospital which is equipped with X-Ray. A Simple X-Ray of the affected region will give the information about any fracture in the bone. The fracture could be simple or complex. Simple fracture is a crack in the bone but no displacement while complex fracture means that the bone has not only cracked but also got displaced. If there is a simple fracture then putting plaster on the area would suffice but if it is a complex fracture then open or closed reduction might have to be done. This is a surgical procedure and is done in the Operation Theatre under Anaesthesia.
The goal in managing fractures is to ensure that the involved limb segment when healed, has returned to its maximum possible function. This is accomplished by obtaining and subsequently maintaining a reduction of the fracture with an immobilization technique that allows the fracture to heal and at the same time provides the patient with functional aftercare.
Open Reduction and Internal Fixation (ORIF): In this type of surgery the site is opened and the bone exposed. The surgeon then fixes the bone and stabalizes it by using plates and nails. The wound is then stitched and a plaster applied. Once the bone has completely healed these plates and nails can be removed by another surgery or can be left inside if they are not causing any problem.
Kirschner wires or K-wires are commonly used for temporary and definitive treatment of fractures. However K-wires resist only changes in alignment; they do not resist rotation and they have poor resistance to torque and bending forces. K-wires are commonly used as adjunctive fixation for screws or plates and screws that involve fractures around joints.
Closed Reduction - In this procedure the orthopaedic surgeon would fix the bone without giving any incision and simply manipulate the bone to bring it into proper position. This is done under C-Arm in the Operation Theatre. C-Arm gives live X-Rays of the site and tells the surgeon that the job has been done properly. After this the surgeon puts a plaster on the site to immobilize it and prevent displacement of the bone till healing takes place.
